term-to-age-80 life insurance policy plus accidental death coverage† and
is available with a $25,000, $50,000, $100,000 or $150,000 benefit.

† Policy Description: The base policy is a modified premium term-to-age-80
product. The initial term period can either be 1,2,3,4 or 5 years in duration, depending
upon issue age. All renewal term periods begin at a 5-year plus one age (i.e. 21,
26, 31, 36 …76) and will be 5 years in length except for the final term period.
The final 4-year period, which will always begin at age 76, will expire and the
policy will terminate at the policy anniversary following the insured's 80th birthday.
The base policy face amount is one-fifth the total face amount of the policy. The
balance of the policy is an Accidental Death Benefit rider. The full benefit will
be paid for all causes of death resulting from a covered accident. The base policy
benefit will be paid for deaths, which are not the result of a covered accident.
The policy pays for all causes of death, except suicide, while sane or insane within
the first two years the policy is issued.
• Exclusions For Accidental Death Rider: This policy does not cover death caused
by: (1) Disease, sickness, bodily or mental infirmity, or medical or surgical treatment
of same; (2) Suicide or intentionally self-inflicted bodily injury, while sane or
insane (reference to insane not applicable in Missouri); (3) Being under the influence
of any drug, narcotic, poison or gas unless taken on the advice of physician; (4)
Service in the military, navy or air service of any country; (5) Participation in
any speed contest; (6) Insured's intoxication (blood alcohol test of .10 percent
weight by volume or higher); (7) Air travel as a pilot, student pilot or crew member;
(8) Committing or attempting to commit an assault or felony; (9) Taking part in
a riot, insurrectionor terrorist act; or (10) Skydiving, hang gliding or hot air
ballooning.
